Instead of trying to find different forms for various LSSLink and Electronic Health Record requests – you can now fill out just one form! IT recently created a form for the most common, straightforward requests that will collect all the information needed in one place.
The new form is the LSS Applications Request form. By filling out this one form you are able to add/update provider info in myUnity, request non-LSS myUnity access, add medication, delete document, and also make an LSSLink request.
The new request form is designed to ask relevant questions for each request providing the necessary information upfront so IT can more efficiently resolve the request.
You can find the new form on LSSLink, under Forms > Most Used Forms or on the IT Welcome Site in QuickLinks. Old forms will be deleted and these links will redirect to the LSS Applications Request form.
