Annual Wellness Testing Happening Now!
At LSS, we are committed to building a culture of healthy employees. We honor that commitment by offering a comprehensive wellness program designed to improve your health, well-being, and productivity, both at home and at work.
As a part of the wellness program, we offer annual wellness testing, in partnership with Interactive Health, which will take place August 31, 2017 – September 29, 2017. Your wellness test enables you to receive the best insurance premiums possible and lets you know if you met your wellness goal from the previous year.
Here’s what you’ll need to do to participate in wellness testing:
- The testing process must be completed no later than Fri., September 29, 2017.
- Fast for 12 hours prior to your testing, but be sure to drink plenty of water.
- Plan and schedule when and where you will test:
LSS On-Site Testing
- Make an Appointment: visit myinteractivehealth.com or call 1.800.840.6100
- Scheduling Deadline: One week prior to testing date.
- Additional Information: Look for posters and communications at your location for dates and times on-site testing will be performed.
Lab Off-Site Testing
- Make an Appointment: Call 1.800.840.6100
- Scheduling Deadline: No later than Friday, September 15.
- Additional Information: Please carefully read all instructions provided in the packet you receive from Interactive Health.
If you are a first-time participant registering online, your sponsor code will be NXJHM.
Watch your LSS email account for specific details regarding the testing process, including important deadlines!
If you are enrolled in the LSS Medical Insurance plan as of September 1, 2017, you (and your spouse, if covered on your plan) must participate in wellness testing in order to receive discounted medical insurance premiums in 2018.
If it is medically inadviseable for you to participate, or you are unable to meet your wellness goals, you may be eligible for a Reasonable Alternative Waiver. You may request a waiver by calling Interactive Health at 1.800.840.6100. Carefully follow the instructions and deadlines provided by Interactive Health. The waiver must be requested and submitted annually, if a chronic condition is present.
